CORNWELL, JACK R. Jack R. Cornwell, age 75, of Waco died Thursday, August 28, 2003 at his residence. A private family memorial service will be held. Survivors include wife, Lyla Riley Cornwell of Waco; children, Kenneth Cornwell and wife, Karen, Linda Scott and husband, John, Sue Evans and husband, Rick, Chip Slade and wife, Valerie, Phil Slade, Kelly Slade-Hardtke, and Amy Slade-Wheeler and husband, Terry; grandchildren, Casey Cornwell, Kristin and P. J. Scott, Nathan Evans, Carolyn and Shelby Slade, Emily Hardtke, Lauren and Riley Slade and Jackson Webb; special loving friend, Nelda Renner of Irving; and brothers, Leroy Cornwell of Pontiac, Michigan and Terry Cornwell of Arizona.
